Big Dawg Diesel & Surplus is pleased to offer health insurance benefits through Blue Cross Blue Shield of Texas. This page contains everything you need to sign up for coverage, including information about costs and benefits as wells as the required enrollment forms.

Signing Up for Benefits

Full-time employees working 30 hours or more per week are eligible to enroll in coverage following their 60-day new-hire waiting period. Coverage will begin the first of the next month.

Your health insurance renewal date is June 1. Please be sure to complete the required health insurance enrollment and/or renewal paperwork before the deadline, April 11, 2025.

Required Forms

If you are a NEW EMPLOYEE, please complete both forms below. If it’s open enrollment time and you’ve previously made a coverage decision, you probably only need to complete the Election Form.

Health Insurance Election Form

ALL EMPLOYEES: please click on the employee election form link, make your plan selection, and sign electronically no later than April 11, 2025. Your new benefits will begin June 1, 2025 and will remain in effect until June 1, 2026. You only need to complete page 2 of the election form (the PCP selection) if you 1) are enrolling in an HMO for the first time and need to select a primary care physician (PCP) or 2) you are currently enrolled in an HMO and would like to change your PCP.

BCBS-TX Enrollment Form

If you are enrolling in or declining health coverage for the first time, or if you are adding or dropping dependents, please download and complete the employee Blue Cross Blue Shield enrollment/waiver form and return it to Henry Lowery. If you are already enrolled or have previously declined coverage, this form is not required.
