Spartacus Consulting is pleased to offer health insurance benefits through Blue Cross Blue Shield of Texas and dental & vision insurance through Humana. This page contains everything you need to sign up for coverage, including information about costs and benefits as wells as the required enrollment forms.

You can also scroll down to find more important details about your coverage, including the provider search tool, member portal login, important notices, and info on free perks. Please be sure to review this entire page so you’ll fully understand your options, rights, and responsibilities.

Signing Up for Benefits

Full-time employees working 30 hours or more per week are eligible to enroll in coverage following their 60-day new-hire waiting period. Coverage will begin the first of the next month.

Your health insurance renewal date is May 1. Please be sure to complete the required health insurance enrollment and/or renewal paperwork before the deadline, April 28, 2025. Your dental & vision insurance renew February 1, so you cannot sign up for those benefits at this time.

Required Forms

If you are a NEW EMPLOYEE, please complete all forms below. If it’s open enrollment time and you’ve previously made a coverage decision, you probably only need to complete the Election Form.

Health Insurance Election Form

ALL EMPLOYEES: please click on the employee election form link, make your plan selection, and sign electronically no later than April 28, 2025. Your new benefits will begin May 1, 2025 and will remain in effect until May 1, 2026.

BCBS-TX Enrollment Form

If you are enrolling in or declining health coverage for the first time, or if you are adding or dropping dependents, please download and complete the employee Blue Cross Blue Shield enrollment/waiver form and return it to Henry Lowery. If you are already enrolled or have previously declined coverage, this form is not required.

Humana Enrollment Form

If you are enrolling in or declining dental or vision coverage for the first time, or if you are adding or dropping dependents, please download and complete the Humana employee enrollment/waiver form and return it to Henry Lowery. If you are already enrolled or have previously declined coverage, this form is not required.
